・尿道異物に対する当院の対応について
腹圧性尿失禁の再生医療:PRP治療
亀田メディカルセンターの泌尿器科は、関係各科との連携により、医療の幅や奥行きが増したと実感しています。治療困難と言われた方でも治療が提供できることもあり、まだまだ、行き届いていない医療のニーズを感じ、当科の役割を果たして行きたいと考えています。
現在では常に10名を超えるスタッフで泌尿器科疾患の治療にあたっています。当科の役割をよりわかりやすくすることで多くの方に安心して医療を受けて頂きたいと思います。特に本邦の中でも専門性の高い医療を受けていただくために、6部門の専門外来を設けました。専門性と各専門性の協調で安定感のある医療が提供できています。

1.泌尿器科悪性腫瘍外来:前立腺癌、膀胱癌、腎癌などの一般的な悪性腫瘍の治療から、局所ホルモン抵抗性前立腺癌の外科的治療、後腹膜腫瘍(脂肪肉腫、平滑筋肉腫など)の原発腫瘍の治療、再発腫瘍の切除などの診断、外科治療の策定、施行をおこないます。また、化学療法、免疫療法や放射線治療は腫瘍内科と連携し治療をおこないます。前立腺がんの手術では尿漏れが少ない術式を確立しています。
2.前立腺肥大症外来:男性の高齢化に伴い、前立腺肥大症、排尿障害は必然と多くなります。また、心疾患・呼吸疾患・脳疾患を合併されている方も多くなります。年齢や合併症を理由に手術を受けられず、カテーテルを留置されたまま排尿管理を受けている方の相談も少なくありません。CVP,TURP,HoLEPの術式をそれぞれの症例に応じ施行しています。抗凝固剤(血液サラサラ)内服下でも治療のオプションが広がります。年齢や合併症などを理由にカテーテルが入っていらっしゃる方は一度ご相談ください。

3.尿路結石外来:尿路結石の治療は上手くいって「当然」と医療者、患者様も思いがちですが、尿路結石治療の合併症は特に注意が必要と感じています。結石の治療の後、尿管が狭くなり「腎ろう」が抜けない。尿管ステントを定期交換しているなどのご相談があります。手術後の敗血症ショックになる例もあり、技術力の高さ、スピード、適切な判断が要求されます。泌尿器科医の中でも、悪性疾患の手術はするが結石などの手術は経験したことがない先生も多く、しっかりとトレーニングを受けた医師の元結石治療を選択されることが良いと思います。 尿管狭窄症や尿管閉塞などの合併症があっても、再度尿管をつなぎ合わせ結石をフリーにする(stone free)に繋げていきます。
4.女性泌尿器科・骨盤底再建外科外来:骨盤臓器脱の治療法として行われる腹腔鏡下仙骨腟固定術(LSC)の普及活動を行い10年が経ちました。まだまだLSCの手術方法や術前術後管理など「低侵襲な治療」とはどんなものかを知って欲しいと感じています。中でも子宮を切除しないLSCの普及も大切です。骨盤底リハビリテーションの重要性などお伝えしないといけないことがたくさんあります。
尿管腟ろう、膀胱腟ろう、尿管閉塞による水腎症に対する診断治療に積極的に関わります。予防方法や診断、治療方法などの教育にも力を注ぎたいと感じています。

5.男性不妊:
6.アクセス外来:人工透析を行うためには1分間に約200mlの血流が必要となります。腕の動脈と静脈を繋ぎ合わせるシャント手術が必要です。バスキュラーアクセスの中で最も一般的なものです。シャント手術は細い動脈と静脈を繋ぎ合わせますが、繋ぎ合わせるだけでなく、人工透析ができる血管を作成することが必要です。つなぎ合わせた血管が細くなった場合は、狭窄部位を広げるPTAを適切なタイミングで行う必要があります。バスキュラーアクセスを長持ちさせる。あるいは最適なバスキュラーアクセスを選択するなど個人にあわせて質の高いアクセスを提供します。
