Items necessary for admission procedure
Please prepare the following items for admission.
- Kameda’ hospital ID card
- Japanese National Health Insurance Card (*only if you are registered)
- Signature stamp (Hanko/inkan)
- Referral Letter (*only if you have received one)
If you have received the following forms before the admission date, please fill them out and bring them with you
- Proclamation of hospital stay
- Patient questionnaire
- Consent forms for treatments
We will inform you of the time you can be in your room prior to your arrival. Please note that sometimes the room and time to ingress to the room may be changed.
For Private insurance holders and out of pocket payers.
