Our Department of Home Care & Community Medicine provides continuous, integrated medical and long-term care services for patients during hospitalization and post-discharge. We are dedicated to supporting regional patients so they can continue their healing and recuperation journey at home with complete peace of mind.
To deliver holistic, patient-centered care, we actively practice multidisciplinary team medicine—combining home medical visits, home nursing, home nutritional counseling, home rehabilitation, and home dental care.

Comprehensive Home Healthcare Services
1. Home Medical Care / Physician Visits
Home physician visits are designed for patients who cannot travel to the clinic due to chronic illness/bedridden status, individuals with intractable conditions using medical equipment (e.g., mechanical ventilators), and advanced cancer patients desiring home-based palliative care.
Unlike traditional emergency "house calls", planned home medical visits require a pre-established contract and thorough consultation regarding treatment goals. Once contracted, regular scheduled visits are conducted.
24/7 Emergency Support System:
Contracted home-care patients experiencing sudden changes in their medical condition receive 24/7 coverage via telephone support, emergency nursing visits, or urgent physician house calls.

- Service Area: Kamogawa City, Katsuura City, Isumi City (select areas), Minamiboso City (select areas), and Kimitsu City (select areas)

2. Visiting Nursing Services
Visiting nurses visit patient homes to monitor medical status from a specialized nursing perspective, providing evidence-based nursing care, medical management, and 24/7/365 peace of mind. We support patients and families across all stages—from preventive care and daily QOL improvement to end-of-life and compassionate hospice care.

- Core Services: Health status monitoring, prescribed medical procedures, physical/hygiene care, pressure ulcer (bedshore) prevention and management, home rehabilitation support, end-of-life / hospice care, and family caregiving consultations
- How to Access: Available to individuals with chronic illnesses or disabilities upon receiving a "Visiting Nursing Order" from their primary attending physician. (Inquire via your physician or care manager)
3. Visiting Nutritional Counseling
Registered Dietitians visit patient homes to evaluate dietary intake, recommend meal modifications based on medical conditions, and provide practical advice on cooking techniques to facilitate easier swallowing and digestion. We also manage tube feeding regimens (e.g., PEG/gastrostomy or nasogastric tubes).

- Coverage & Frequency: Up to 2 home visits per month covered under Long-Term Care Insurance ("Home Medical Management Guidance") or Medical Insurance ("Home-Patient Visiting Nutritional Guidance")
4. Home Rehabilitation Services
Rehabilitation therapists visit patient homes to assist individuals living with physical limitations or functional impairments. Beyond physical function maintenance and exercises, our therapists adjust living environments (recommending assistive mobility technology, welfare equipment, and home modifications) and train family caregivers.
5. Home Dental Care
Specialized visiting dentists and dental hygienists provide professional oral hygiene, denture adjustments, and specialized oral care at home for patients who cannot visit a dental clinic.
